Personal Loan

I prevoiusly had a personal loan which has being paid up and closed. I requested a paid up letter at the Adderely Str branch in Cape Town. My account was at collections as I was previously under debt review. I have as yet not recieved the requested documentation. The reason for this request is due to being told there is an outstanding balance when checking my credit status via Transunion credit bureau. I queried this with them and got the same response - account in arrears. Can someone please let me know what is the hold up in getting the requested documentation and also why does this account reflect in arrears if it is paid up and closed?
