Double accident benefit that was not paid

My late father was having a hollard funeral plan police for more that 5 years, when he pass away I call hollard to find out what was a process, they told me as death was due to accident they need all document and accident report and a drivers licence as he was driving, so I provide them with all documents and they only pay death benefit and they told me that accident benefit was going to be procces late, after few weeks they told me that Der was no death benefit, I ask a prof of that contract they say evrthing is voice recoded and I ask for a copy of recoding they say they can't provide me with that. Even now I don't have a answer. I dd'nt believe when people say hollard is not good till its was done to my family.
