Incorrect debiting of my account ref: cc6407017

On 23/01/2014 I sent the service centre an email saying that my account was debited with an RA premium of R558 that I don't know of. I also sent them bank statement showing the debits. I have signed for an RA for R300 not R558 . After sending an email I heard nothing from them till I called my financial adviser a week later saying there hasn't been any response. Then they responded however they insisted that it was my RA, my RA for R300 was not deducted for Dec 2013 but was deducted on 9/01/2014 and the Jan 2014 premiums on 21/01/2014 . Problem is on 28/12/2013 and 21/01/2014 premium for R558 was deducted saying it was for my RA although R300 for my RA was deducted twice in Jan 2014 for my Dec 2013 and Jan 14. It seems like this RA for R558 has the same account number as mine and loaded with my bank details however it does not appear under my portfolio when I check all my accounts with OM online. After 2 weeks the money has not been refunded and they are unable to see that imam being debited for 2 RAs, I also believe that the contract name on the R558 RA is not mine however the staff is unable to figure that out. It's worrying at the level of service and competency, DISAPPOINTED.
