Unauthorised bank account debiting

I renewed my Vodacom cellphone contract through Elite Mobile and they added insurance on my account when I very clearly told them that I do not want insurance. I have been seeing UNAUTHORISED debits on my account every month. It's essentially and this company will be reported to FICA/FSB or whatever body governs these and shut down. I want my money refunded immediately!
