MFC hired to deal with complaints!!

Can you please explain to me why you have set the same debit order to go off twice? On the 2nd Jan'14, an amount of R3472.97 ref: Ned/mfc agreed 44731210001 went off my account, which was what we had agreed on, Now on the 3rd Jan the same debit order went off again and this time I didn't have enough funds on this account, as I thought I was done with my debit orders. Now, what I want to know is, Who will be paying the R138, my bank had charged me as an unpaid debit order fee, and this is certainly not a mistake from my side? I also need MFC to provide me with a letter stating that this was a mistake from their side, as we all know unpaid items. Have a negative impact on one's credit rating and I certainly refuse to pay for sins I haven't committed. I'm certainly not being unreasonable when I demand MFC pay back R138&a letter or else on monday my first task is to contact thhe ombudsman..I really curse the day I applied for finance with MFC
