Two people required to collect a cheque book

The other day, to be exact on the 18th January, I had the displeasure of having to go to the bank to collect a new chequebook. This is a chequebook for our church, and obviously, two signatures are required for this purpose.

I completed the application form for a new chequebook, and two persons signed this application. On the reverse side of this form, myself and the other signatory authorised me to collect the chequebook. The exact words of this authorisation are:
I/We the undersigned hereby authorise the bank to hand my/our chequebook to... who will identify himself/herself by means of RSA identity document or passport number... and will sign as follows...

After handing in this form, I was made to wait a very long time, before the clerrk came back and informed me that she could not hand over the chequebook, unless the second signatory was present!. I asked to see the manager, thinking that she at least would know standard operating procedures. To my surprise and disgust, I soon realised that this instruction came from her! She was adamant thaqt both signatories had to be present to collect a chequebook!
