Excuses to keep debiting my account

I have made a previous complaint on Hellopeter regarding my failed attempts to cancel a policy that was sold to me under false pretences in November 2012, was promised that the matter would be rectified, and that the premiums that were being deducted would be refunded to me.

Up to this day Momentum keeps on debiting my account and I am given excuses such as we will call you back, sorry I forgot, and the fax number we supplied is out of order for the last four months.

Please supply me with a number where I can have this matter investigated and sorted out please, or please supply me with the contact details of the insurance ombudsman.
