Cannot deliver on promises

On 25/05/13 I paid the balance on my Amex credit card and decided to close the account. I contacted their call center and was informed that I should have called them for a settlement amount as the amount given on the self-help service line isn't the actual total. I paid the new balance and was informed that the EFT would only be cleared by 29/05/13, and I should call back then to have the account closed.
I called back on 29/05 and as I was put on hold for quite some time, the call center agent said he could get someone to call me back within 2 days. I told him that I want the account closed and I don't want any fees accumulating while I wait for someone to call me back. I was told that this wont be the case and was given a reference number 63036369.
A week later (05/06/13) I got a call from them and was persuaded into keeping my account active on condition that I will pay no fees for 1 year. I was told that the ref. number for this call would be sms'ed to me, but it wasnt.
On 11/06/13 I received my statement and there is a balance of R11.39.
I know it is a small amount, but the fact is that I was to. I would like this amount to be reversed and my account closed immediately.
