Not cancelling policy as requested twice

We have asked mr naicker twice to cancel the policy first time in march second time in april but still the premiums are being deducted DOUBLE each month and then we still get acoounts that stated we are in arrears and the 3rd time we phoned mr naicker he said he will reverse the debit order which he didn't do!!!
