Changing amounts on contract using my signature

In mid 2012 I bought 8 additional points on my THC package. The consultant and I negotiated on a specific price, and on my yellow copy (the copy which is given to the client) of the extended upgrade it was written that I have paid. Afterwards the consultant called me stating he made a mistake and that I should pay R500 extra, which I did. After this a lady called stating that there is additional membership fees payable by me. I then replied that she should cancel this deal as I am not willing to pay any additional fees.
I received a blue copy (THC's copy of contract) of the extended upgrade in the post which is totally different from my yellow copy. My R650 and the words "Paid" which is stated on my yellow copy is now suddenly scratched out on the blue copy with my Signature falsely added next to the scratched out number. The false signature is clearly differently from my original signature on the same paper and it is even done with a thicker pen. They also stated on the Blue Copy that my Membership Fee for the upgrade is R1238.48, this also does not correspond with my Yellow copy.
I have clear evidence of this and I am going to open a case of. I am utterly shocked by this.
