3 months still not cancelled

I took out a contract in january via the phone which i was promised had skype. after many conversations and several different ppl it was collected. 2 weeks later ram tried to deliver it again and i refused to accept and after several more conversations agreed i would cancell the debit order and be refunded my money. several more conversations and they still have not found the recording where i was to about skype or refunded me. i have mailed, logged enquires, had the phone put down on me etc etc. 2 weeks ago i am phoned about my account being in arrears! for a phone that was picked up 3 months ago!!! i find this totally unacceptable and again am being bounced around from operator to operator! how can it take 3 days to open a contract but 3 months later i am still waiting to be refunded and have it cancelled!!!
