Unauthorized debit orde

Today I received an sms from my bank that MTN debited my bank account with the outstanding balance of my MTN account which I already paid on the 25th. There are three things that bothered me about this. Nr 1 I have been a cash paying customer for close to 5 years, paying my account via eft, Nr 2 I didn't authorize ANY debit order from MTN. Nr 3 I didn't give my bank account details to MTN, how on earth did they get my details???!!!
The person at the Service provider says that when the transfer of the contract to my name was done in March then I gave them permission, but I remember as clear as day that I ONLY marked the way of payment as EFT, and left blank all the bank details. I want an explanation of the my three queries and a refund of the extra payment ASAP.
