Faulty Z10 OBF, horrible service & no delivery

Dear Vodacom,
I am currently livid by level of service and broken promises (contractual promises):
I recently attempted to upgrade our contracts from the BB Torches to the BB Z10's for both my wife's phone. It took 4 attempts online, 2 store visits and an online chat to get serviced!?
Did NOT receive the HDMI cable NOR Shell cover as contractually obliged
My wifes phone speaker did not work out the box (defeats the purpose of the phone)
Store could not help, chat could not help, spoke to Wayne (fine), Khanyo Mjungu (terrible), Olga Mutsumi (Team Leader - completely dis-empowered) to assist with replacing the phone and getting the accessories.
My issues are as follows:
EVERYONE passes the buck as it is NEVER their fault or problem and they could care less if you are sorted out
It took me over 6 hours in total to have NO idea when the issues will be resolved
Vodacom KNEW that they were sending out incomplete orders but have not informed any clients.
Vodacom refuses to cover the risk while I don't have the shell cover
WE offered a solution BUT YOU aren't able to allocate me the items from a VODACOM store!?
Cannot log a complaint with a team leader
Unfulfilled contractual obligations
