Not signed a contract but get charged

Hi, I have phoned your call centre on 2 occasions about a penalty that was charged on my account for "early contract termination". The lady at the call centre who I spoke to said that they will have to listen to the call / conversation and that they will phone me the next day, which never happened. I have not signed or asked for a contract. I wanted the month-to-month 4MB bundle deal that Telkom offered because I already have a modem etc, so I don't need / want a 24 month contract and get a modem etc free. Please look into this and rectify the account and remove the penalty because I did not ask for a contract. I said that I want a month to month. Also, my telkom line was also off for about 3 weeks during December 2013 - January 2014 (line fault). This should also be credited because I could not use the line for that period and I had to buy 3G internet bandwidth during that time. Please can we get this all sorted asap. Thanks, appreciated.
