Losing my claim forms

The shop lost my claim forms of which it included my ID copy with case number. All personal particulars was lost where today's criminals can get access to my ID doing activities. It was handed over to Dane from Clearwater branch to hand it over to Carol but got lost in between their transaction but even to date no response from them claiming that I am responsible for the loss that took place at their facility.
