Not Providing a service

My Phone got last week Thursday (2014-01-16). up until know i have not yet received a black list form from them after they assured me that they will e-mail it to me on the next day. ( It is now Monday 2014-01-20)

When I went there Today they now said that I had to phone in in order to get the form.
I then went home and phoned cell c they then said that I the store was suppose to have sorted it out in the store, cell c then promised that they will e-mail it to me. but i have up until now not received any thing.

All I want is the form so that i can settle my insurance claim. and get a phone again as i am stranded with out it.
