Telkom Adsl line and phone line

I cancelled my Adsl line and telephone on 5 October 2013 (Telkom says 10 Oct) The ref no is coc or toc46707339 and I also wanted to convert back to pre paid.
I received an account today for adsl line and tel even/week ender.

I also phoned today to report the matter and the lady advised that the cancellation was on the system and she does not know why it was not cancelled. She also said that she would phone me back today and advise me of the cancellation. I also want to cancel my phone line now due to all the problems with Telkom.
I tried to phone again from 16h05 (17h32) and kept waiting for an hour and half with no consultant available. I had to hang up and decided to write this email.
Can you please help me to reslve this problem.
I'm so sick and tired of Telkom's promises that I now also wnt to cancel my telkom phone line.
