Incorrectly placed on ITC

This seems to be the only way to get an answer out of Vodacom, so would the person who attends to this please carefully read my request and respond to each point.
I have received an unsatisfactory response from your legal department and need a response to the query below (please look into the history of this case as well, and confirm when this ITC record should have been removed.

Please can you confirm why this was placed on my record in the first place, as this entry remained there from 2011 until now, and as you state, the amount in question was paid in 2011.
This was also the matter of a dispute between the branch and myself, and no-one has to date taken any accountability for the wrong information that was given to me at the time of closing this account.
I know that you are the biggest cell phone provider, but this experience has just left an extremely negative perception towards Vodacom due to the
(i) the misinformation
(ii) lack of accountability on your organisation's part,
(iii) the manner in which this was dealt with,
(iv) and the fact that after all that, your company had the audacity to place me on ITC (incorrectly as noted, as I settled the disputed amount)
