Autopage reconnecting mobile data services

After incurring a huge bill as a result of out of bundle data usage during December (2012) and no means to control data usage, I cancelled all mobile data services in January 2013.

Recently I have noticed that I am paying for mobile data services again. I held back some payments and logged a formal complaint. My cellular service was suspended on a weekly basis recently and I have made a number of calls to Autopage. Every time I was met with incompetence. I seems that the call centre is unable to grasp the dilemma.

I demand the following:
* Un-suspend my cellular account.
* Cancel all mobile data services again (like requested in Jan 2013).
* Refund me for all payments I made this year for mobile data usage that I never subscribed for.
* Refund me for all the re-connection fees.
