Cancelled Account - Still being Billed

So I cancelled my account on the 27th of July 2013, have been on the phone with Telkom consultants almost 10 times already trying to ensure that the Debit Orders stop and my account is closed, they all have assured me they will stop. Then end of August the Debit Order did go off which shouldn't have as I gave more than 30 days notice of cancellation. Phoned and they said they will refund me, still nothing happend. Now I received an e-mail with my Invoice for September showing that my account will again be debited, in other words my account still not cancelled and they still haven't stopped. How can I get my account cancelled and get them to stop debitting my account for a service I no longer use???
