Ghost Upgrade!!!

I called MTN of the 11st August to cancel and existing contract as I felt I didnt need it anymore and it has been due for an upgrade for over a year... only to be told the contract had been upgraded a few weeks prior. The call centre gave me the name of shop where the upgrade took place as well as the person (ie Shameem Ramjee) who handled the upgrade. I called only to be told a drivers licence with my name was presented for the upgrade and they didnt bother to ask for ID or proof of residence even though it had been over three years since the contract was first taken before doing the upgrade. Having sent Shameem all the documentation she required (my ID, my drivers license, my proof of residence, my affidavit plus audit proof of the dates when I was in CT reflectng my daily itenerary (as I am based in Sandton) to proof the upgrade - there has been nothing done on my account and now I have just received statements for the upgrade I didnt do! my feeling is that she was involved - how do you upgrade an account without proper identity and proof of address, proof income etc. this has to be an internal job and I suspect she would know about it...
