Such a struggle for insurance

I have been trying to get insurance for my phone for the past 3 or so weeks, to no avail. I was given a form to fill in at one of the branches and was told that I would have to fax it through to the insurance fax line, after 24 hours, I would call the insurance line to confirm receipt of my application and the machine confirmed that the form had not been received, to which I then held on for an operator, after holding for about 20 minutes, the line cut. This happened three times, and with each time I would send the form through again. Eventually, I called through to the main call centre to enquire and, twice I was told that the insurance line was down, but that my details would be sent through to a consultant that would in turn call me, I have waited two weeks for this.
I then went to the Cresta branch, as advised, and there, the consultant sounded very promising, and to date, I still have no insurance against my phone and neither has anyone contacted me in this regard, even the supervisor failed to call me back.
This is very poor service. What if something happens as I wait on MTN?
