2months & my address still not updated

I have had an account with Vodacom for years. I went into Bayside at the end of April to a) upgrade a phone and b) change my address. I have been waiting for my statement ever since, espcially as, given the upgrade, I'm now not sure what my bill amount is.
I went into Vodacom on Saturday to ask why it was taking so long - I didn't get more than a meter through to door and Erin asked me how she could help. I explained I was still waiting for my bill and she said it shouldn't take that long. She was quick to say 'we just send the form off to HQ, if it gets lost along the way it's not our fault'!!! I hadn't even asked if that's what happened and she already started playing the blame game! This is an aweful approach to customer service! Of course it's your (as a company) fault! She said you can fill in the form again - but given that you lost the first one - this seems pointless!!
I just want you to send my bill to the address I've given you. If you don't send me the bill, I can't pay you!
It's very, very simple! Update my address please, and maybe ask Erin to complete a customer service course - she clearly needs it!
