Debit orders still active 7 months after cancelled

I cancelled my 8ta contact after 24 months in December 2012.. I completed a form in the store and
Also faxed the form to the department responsible. Despite 7 emails requesting them to cancel contract and stop debit order, it still hasn't been done and I still pay every month as I can not cancel the debit order!!!
