Illing me after cancelling my month to month cont

I was on month to month contract from 2011 and in october 2012, i decided to end/cancel the contract.i went to Brackenhurst branch with my friend whom we joined together in 2011. the lady gave us the forms and we filled in and give them back to her hoping to get positive results the following month.we were expecting last bill in november. So to our surprise the payment went throug even in December, thets when we realise that our contract is not cancelled.
Myfriend went there in J anuary the 7th 2013 and explain the situation to themthey did the follow up on her account and sorted everything on the spot and told her that they will refund herShe got her refund on the 10th
.on the 8th when i go there the same lady told me that she cant find my form.but on a system it shows that i cancelled in december, I asked to see the prove for that december and did not give one and said to me she cant find it either.she will look for it and call me tomorrow[friday the 11th].
she called me on that friday to tell me that she still cant find my form.she said to me i must cancelled again and im not going to do it.
i cant afford to suffer /loose my money due to their neglegency.how
