Cellphone Contract not cancelled when due

I am writing on behalf of my grandmother, Judie Blignaut. She is 73 years old and requested more than once that her Cell C contract should be cancelled when the 2 year period is done. She filled in the cancelation form last year sometime, but it was not submitted by the branch. She filled in the form again last year December month, and still the contract was not cancelled. Meanwhile the debit order goes off every month from her bank account. Seeing that she is a pensioner and every penny helps her to survive, it does not help her to make payments for something that she does not even use anymore.
She and my mother went to the branch in Mokopane to complain, but it does not seem to help. When people from the Polokwane branch phoned her, she told them as well that the contract should be cancelled.
I personally had problems with this branch last year and HalloPeter helped me to solve that issue. Now I hope that you can help me again with this issue of cancelling the Cell C contract for my Grandmother. Not only that, but we request that all the money that was taken from her account form the month after the last payment should have been subtracted, paid back into her account.
