Black Tag Debit Order not taken and I have to pay!

I have recently gotten the Black tag membership. all cool and working great, BUT there are 2 problems.
1. when I took out the black tag contract I stated and provided my banking details so the amount can be taken by debit order, to my surprise this never happened and I received an sms saying my account is in arrears, I phoned the gym, gave them the ref number and they never phoned back, so I went there myself and there is some problem with the black tag finance or something, So thanks to your problem I had to pay a penalty fee, This is TOTAL BS! why must there be so much problems and excuses??? I will not go in again and pay cash at the gym, I asked it to be taken off my account with a debit order and that is what needs to happen, please reverse the penalty fee to me and resolve this.
