Activation fee deducted twice - not refunded yet

When my fiance joined Roodepoort Virgin Active he paid his activation fee at the gym with his debit card. On the 7th of June his debit order plus another activation fee went off his bank account. On the 12th of June he advised the branch of this problem and they said that the money would be paid back before the 14th of June. It was not paid back so he returned on the 18th and once again he was promised that it will be in his account the next day. Today is the 20th and we are still waiting for the money. I think it is very unprofessional of Virgin Active and unnecessary that he has to beg for money that belongs to him. Can we please get the service that we pay for?
