Cancelled policy but still billed for it monthly

I cancelled a funeral policy with Miladys on the 30th of March 2013. It was never cancelled. I was charged for it even afterwards on my account. I went to different stores to make sure that its cancelled, but everytime I went to the store, it was mentioned that we need to re-fax the documents for cancellation. I have gone through the same process for more than 5 months. I get phone calls from their collections department almost every 3 weeks. I have explained my situation to different people and ended up speaking to Ashika who promised to resolve the matter. Up to date nothing has been done and I still receive phone calls from them.
Its obvious that somebody didnt do their job, from the onset of my cancellation of this product. I cannot stand being harrased by Miladys anymore for their mistakes
