Medication incorrectly dispensed for my toddle

I have a 2 year old who is ill. I went to the pharmacy and purchased cough syrup, suitable for my toddler. When I wanted to check the dosage to dispense I noticed the medication clearly noted that it was not suitable for children under 6, unless prescribed by a doctor. They kindly refunded me for the goods purchased, but did not provide a satisfactory response as to how the goods were dispensed incorrectly, nor what corrective action would be taken to ensure this situation did not recur.
