Injustice after my parents deceased

My mother BB Sithole ID 6409210860083 who was a teacher for as long as I can remember passed away 11 May 2008 and shortly in 17 December 2009 my father passed away. Ever since my father's passing I have tried everything in my power to lodge a claim with the Department of Education for my mother's pension, but have been unsuccessful in everyway. All I get is the ups and downs, this lady does not even phone me to update me, everytime I phone GEPF in Pretoria they tell me the forms were returned because they were incorrectly filled in, sometimes they wanted my ID copy which I had included along with my parents copies, this last time which was in June they told me that the forms were returned because they did not fill in my mom's notch for the year she deceased. I mean honestly who hires these people because clearly they don't even know how to complete all these documents. My forms have been doing these ups and down for the past 3 years and unless I phone Pretoria to find out, nobody cares to phone me coz clearly they know it is their faults. I have tried to report Mbali to her supervisor Norma but have not succeeded there as well as I was told the GEPF people sometimes loose forms.
