Payment not made clientele forms needs R350 paymen

My 91 year old mother passed away on the 28/07/2013. I sent Dearth certificate, mortuary form with hospital report and Dr signature, my ID, Bank statement and the diseased ID copy on the 30/07/13. Clientele told me on the 01/08/2013 when I phoned, that they received my documents they will call me, which they never did. after the funeral I called they sent me forms to fill in which needed a police signature and the Doctor to fill in. I took them to the Hospital they told me they need medical history from 2005, my mother was never to a doctor she was highly christian, I took her to the hospital on the 27/07/2013 ahd she passed on the 28/08/2013. I am stuck with that and clientele wont pay without those forms. on the other hand the hospital (Leratong) needs R315 for those forms to be filled in which I don't have.
