Medical Aid approves medical aid tarriff but

When being admitted to hospital for a procedure, this is normally approved by medical aid and established that costs will be accepted at medical aid tarriff. It has now happened 3 times where not once anaethetist informs patient that charges will be at his rate which is higher than the allowed medical aid tarriff. So one goes for procedure believing that medical aid will cover all expenses until months later when one is bombarded and threatened with accounts for outstanding fees which are the fees left over after medical aid has covered their portion. I have one account going back to 2011 which they are still bothering me with which I never received an invoice for except on the occassion that I got an SMS informing me that I have to pay - is this acceptable?

I further notice on the statement sent to me as requested that the breakdown of expenses include emergency procedures - which there were none of, that the total amount outstanding of R1222 does not include the settlement by medical aid of R1116 although they made me pay R1222 to avoid legal action.

They do not respond to my return mail.
The exact same thing is now happening to my daughter who was also not informed
