Not getting maximum benefit

I have done blood tests with my Doctor earlier on. All the requirements on the blood tests as required by vitality were done and duly submitted for my points. All points were allocated accordingly but one benefit of maximum of 25% at Clicks was not activated. The system shows that i qualify for 10%.
In addition, the Pic n Pay benefits are activated on the system and reflects 25% but i have never received any of that 25% payback. Please investigate on the 2 matters and advise.
