Does not want to complete chronic illness forms??

Need to get my medication onto my medical aid's chronic illness benefit, however it seems as if the doctor is unwilling/unable to do this. Phoned about a week ago to ask for this to be done, was promised they will let me know when the form is ready. Heard nothing, so I called them. Now I have to see the doctor again - after I went for blood test a week ago (and saw doctor about three weeks ago) Doctor called to confirm that my medication is working, according to the bloodtest. So why on earth do I have to see him again?? Probably so that they can invoice my medical aid for another consultation, and make money off me... and in the meantime I have to pay out of my own pocket for my medication...
My husband's medication had to be changed, as his current medicine was not working, according to the bloodtest. Now my husband must also go in to see the doctor again, so that he can complete a new form for the new medicine! (My husband saw the doctor about two weeks ago)
We are absolutely not going in to see the doctor again. All the info he needs for the chronic illness forms is in our file, so I see no reason to see him again. Please just fill out the forms!!
