Bringing customers into disrepute

This morning I was called by Daily Morgan (apparently a debt collector of sort) and was told that my account, at Ingrid Takis-Bishop Physiotherapy at the DD Campus in Bryanston, was handed over for collection. I had paid my account one month after visiting the physio 2 years ago (01 November 2011).
Since then this is the 4th time that someone has contacted me on behalf of this inept business and tried to extort money from me.

To quote Ingrid Takis-Bishop's account department from 6th Feb this year:
"I highly apologize for the inconvenience caused. I've just posted the outstanding amount and your account is sorted now. Have a wonderful evening! - Shirley"

That was the 3rd time I had already reconfirmed that you, your secretaries, and accountants are suffering from competency failure... and I've just done it again for the 4th time.
Ingrid Takis-Bishop, I expect a formal apology from you, on your business letter head, for bringing my name into disrepute, for wasting the debt collectors time, and for wasting my time yet again due to your head-in-the-sand approach to running an establishment that is very close to being Boswell Wilkie Circus' best competitor.
