Apparent cancellation of my membership

Beginning of June I received a mail from my medical aid consultant that I was in arrears, I've been making my monthly payments every month, since then the consultant was investigating the "arrear" amount sending me several emails asking for payslip, bank statements. Today after waiting 2 weeks for feedback I managed to get hold of her and apparently my membership was already cancelled in April how is that possible that the consultant doesn't even know what's going on
