Repeated failure to process claims accurately

My mom has been receiving weekly physiotherapy benefits for treatment of a stroke as a PMB. However, it is now the 4th occurrence that Medihelp have rejected the practioner's invoices despite the PMB authorisation being loaded on the system and pertinent details reflected on the invoice.
I have phoned in 3 times and Medihelp have correctly rectified the 3 claims. However, this is getting ridiculous. This will now be the 4th time that I have to phone in regarding another claim that is not being processed against the relevant PMB authorisation.

Previous claims that were captured incorrectly and their service dates are as follows:
268800914 (2013/04/11)
268215817 (2013/03/22)
267153017 (2013/02/15)

This occurrence: 269180638 (2013/04/25)

I request that Medihelp get their claims department in order so that further occurrences of this nature do not reoccur in future.
