REFUND

After been given the wrong information by the Customer Service Department I went and saw a dentist (which was not on the approved list). I was not contacted directly, instead a voicemail was left giving me the right information, which I NEVER got. I am wating since December 2012 for my refund after they have agreed to refund me.
