Builders did not closed the account as promised!!!

I received another account statement in the post showing the account is still open, I reqeusted for it to be closed. I never even fetched the card since 2011??? Please update my ITC that I do not have an account with builders anymore and I want a letter emailed to me as proof that the account has been closed and settled.
