Completely wrong order after several confirmations

I went to this McDonalds in the morning to pick up a Mega Mac meal and two Sausage & Egg McMuffin meals. I checked the order once at the speaker and again at the first window. I then had to wait around 15 minutes for my order, by which time I was late. When I arrived at my appointment, my colleagues were dismayed to discover one Sausage & Egg McMuffin meal and two Egg McMuffins.

How could anyone get an order so wrong after more than one confirmation? I was friendly and patient with the staff to ensure that I got what I needed, but even that was not enough. My colleagues confirmed that this McDonalds is notoriously awful.
