ACCOUNT KEEP GETTING LOCK FOR FICA

I am tired of having to supply FICA documentation every second or 3rd year to ABSA, I have done FICA in the past and the documentation has gone missing so have to do it again. Now all of a sudden this year account lock again wanting FICA documentation. Please advise on what basis this account keeps getting locked
