The account suddenly went FICA locked with no warn

MY small business account has been operating for over 10 years. Last year I submitted new FICA documents due to a change in bank signatures. Today the account was locked because of missing or incomplete FICA documents - a whole 12 months later. How can this just happen on its own? Six months ago it happened to my other ABSA business account. Who at ABSA is deleting all these documents? I spoke to the Rivonia branch and once again sent them all the documents I had submitted last year. The lady undertook to fix the problem. Nothing has happened and the account is still FICA locked. If my tax is paid late will ABSA refund me the penalties?
