Replacement card fee illegal?

On 24 Jan 2014 my Autobank card was swallowed by the Standard Bank ATM machine at the Edleen butchery in Kempton Park. I called it in and was told that the card will have to be cancelled and I must get a new one. I went into the Festival Mall branch on 28 Jan to get a replacement card. I had to sign a form which entailed fees for a lost card. Nowhere on the form did it stipulate an amount. There was a block to tick that I (the customer) do not accept the fees (or something to that effect). I ticked that block.
Now I see that I am charged R80 for a replacement card. I have a serious problem with this for more than one reason. Firstly, YOUR machine swallowed my card. I did not cause the "loss" of the card. Secondly, I ticked that block on that form in the bank. I want my R80 back as soon as possible.
Is this not against the Consumer protection Act as well - making me sign a form for unspecified amounts?
