EX HUSBAND OPENS CREDIT CARD ACC WITH EX WIFE BANK

My ex husband was granted a credit card from FNB using my bank account number. No authorisation was given and I want to know when and where the account was opened. I sent an email to FNB 29/01/2014 and they replied to say that they are looking into the query. The money has been reversed, however nobody has come back to me saying that this issue has been resolved. I don't know if the details has been changed yet. I want to know how it was possible for him to open an account using my banking details. They should not be charging me any bank charges for thier mistakes.
