Un-Authorised Debts - Again and Again and Again

I have notified the bank on numerous occasions with regards to a company that continues to debt my credit card without authorisation, the bank reverses the debts but then allows the company to debt again the next month.
They tell me I must submit forms each time this occurs.
How come I must do all the work and they just let this company get away with continuously?
Currently there are debts going back 16 months that have been picked up, I am told they will review the matter and I should expect a response some time int he New Year... So currently I am almost R5k out of pocket!
The bank is happy to take my money but ignores these illegal transactions.
