Struggling to cancel policy

I have been requesting MiWay to cancel my policy for atleast 3 months, it got so bad that I personally came to your Midrand offices to ensre the cancellation, yet month in, month out I get notification informing me that there was an attempt from MiWay to debit my bank accunt. PLEASE CANCEL THE POLICY!!!
