UNLAWFUL AND DEBITING!!!

My Husband spoke to Givanche from your call centre re an unfamiliar and unauthorised debit order that has been debited from his bank account for cell phone insurance. Givanche informed him he had agreed to this insurance policy, telephonically, during January 2011, which he immediately disputed and requested proof of the recording. Givanche contacted him on Wednesday 2 October 2013 and confirmed that he did not agree to this service and will be refunded on 10 October 2013.
We now urgently request the following:

1. a detailed credit note for the refund, clearly indicating what the refund was for;
2. a detailed schedule evidencing every debit order from his account. The refund did not come close to R57 being deducted from his account for 2 1/2 years.
3. to be refunded for penalty fees incurred as a result of the debit order not being debited successfully. Penalty fees would not have been incurred had you not and unlawfully debited his account.

Withdrawing money from an account without permission is unlawful. Debiting an account on the basis of a non existing agreement constitutes. We will not hesitate to take this matter further.
