Non payment of disability cove

I was involved in an accident in 2012 resulting in leg amputation below knee. Neither GLIS nor SANLAM to whom I have paid premium to cover me for disability wish to meet my claim and currently I have no income. It seems they took premiums under false pretences. All docs supporting claim sent on 2/09/13 - still no action
