Cancellation of policy by Outsurance

I hereby wish to formally request your assistance in dealing with my problems that I encountered with Outsurance. The details are as follow:
1. My policy number with Outsurance was OT8121792 and that of my husband was OT12864834. These were two different policies under different names and premiums for each were deducted from our individual bank accounts.
2. The policy that was cancelled by Outsurance is that of my husband, Mr. Lesedi Edward Piki. The cancellation letter received on the 26 June 2012 was that of my husband, OT12864834. I have never received any letter from Outsurance indicating that my policy (OT8121792) was cancelled. Outsurance used my particulars without our consent on my husband's policy way after we applied separately for our policies. I then appealed this mistake and Outsurance out rightly admitted their mistake.
3. The only letter that I received from Outsurance on the 26 June 2012 was the No Bonus claim and History letter for my policy, OT8121792.
4. To date I have not received any cancellation letter from Outsurance on my policy OT8121792.
5. My last claim was on the 11 March 2011 but my policy OT8121792 was cancelled in June 2012. Why?
